Michael Sean Polk was born October 27, 1975 in Wagoner, Oklahoma. He worked as a grounds keeper for Sequoyah Golf Course and was a member of the Elks Lodge. Michael was the lead guitar player for the band, "Just Cause" and he enjoyed taking care of his dog, Gavin. He was preceded in death by his brother, Terry Don Polk and his step-father, Richard Bauer, Sr.
Michael passed away Monday morning, May 16, 2016 in Tulsa, Oklahoma at the age of 40 years.
He is survived by his mother, Vickie Bauer of Wagoner; his father, Gary Polk, Sr. of Louisiana; his daughter, Haley Breanne Polk of Tulsa; his siblings, Gary Polk, Jr. of Wagoner and Monica Belic and husband, Stephen, of Broken Arrow; two nephews, Christopher Belic and Ryan Polk, his niece, Lisa Belic and great niece, Thea Belic.
A "Just Cause" Celebration of Life will be held in the home of his mother, Vickie Bauer, at 2:00 PM, Friday, May 20, 2016
Arrangements and cremation are under the direction of Hersman-Nichols Funeral Home, Wagoner.
